    FACTS is a project of the University of Illinois and other institutions and is funded by the Office of Special Education in the U.S. Department of Education. FACTS publishes informative booklets for parents and professionals on transitions for young children. You will find these subjects at the web site:
    • Facilitating Inclusion in Community Settings - Creating the environments that supports the communication and social interaction of young children.
    • Writing an Interagency Agreement on Transition - This booklet focuses on the role of interagency agreements in facilitating the transition of young children and families from early intervention services to preschool services as the child turn three years of age.
    • Family and Child Transitions Into Least Restrictive Environments - This booklet is designed to assist families in preparing for the transition from early intervention services to preschool or other educational services at age three. It also has worksheets for parents to use in preparation for their childs' transition into least restrictive environments.
