    The bulletin contains information related to regulations, transition planning process, the transition planning conference, referral considerations, transition activities, and resources.

  2. Agreement between the Department of Health and Family Services (Birth to Three) and the Department of Public Instruction - Infants and toddlers with Disabilities and Their Families Public Awareness, Child Find and Transition Agreement

    This interagency agreement between DPI and DHFS demonstrates a commitment to providing collaborative multidisciplinary services to infants, toddlers and young children with disabilities. A related agreement exists between Head Start, DPI, and DHFS specific to child find, transition and the collaborative delivery of services to young children with disabilities.
