Set.. Go: Ensuring a Smooth Transition from Birth to 3 to Special Education
(Overview). This presentation was developed collaboratively by the Department
of Public Instruction (DPI) and the Department of Health Services (DHS). It is
based on the transition initiative called Ready.. Set.. Go.. Transitions and Options.
In this webcast strategies are shared for ensuring a smooth transition from Birth
to 3 programs to special education.

Set.. Go.. Building the System Between Counties and LEAs. This presentation
was also developed by DHS and DPI. It was used at Regional meetings with counties
and school districts to help them understand the new Program Participation System.

improve transition practices and assure compliance with OSEP indicators, DHS and

Wisconsin's new data collection
system, Program Participant System will improve data collection and support Wisconsin's
compliance with OSEP Transition Indicators. School districts can access the system
by linking to the DPI

data by determining contributing factors or root causes related to data concerns

Transitions addresses both Part C (SPP Indicator 8a, b, c) and Part B (SPP Indicator
12) . We are also looking for comments to improve the document. investigative

Early Childhood Transition Regional Meeting Planning Worksheet can be used to
identify and maintain communication between the county and LEA related to PPS
security coordinators, notice, transition planning conferences, referral, and
on-going planning. This resource could be an attachment to interagency agreements.

of Public Instruction - Infants and toddlers with Disabilities and Their Families
Public Awareness, Child Find and Transition Agreement

This interagency agreement between DPI and DHS demonstrates a commitment
to providing collaborative multidisciplinary services to infants, toddlers and
young children with disabilities. A related agreement exists between Head Start,
DPI, and DHS specific to child find, transition and the collaborative delivery
of services to young children with disabilities. - Agreement
between the Department of Health Services - Birth to Three, the Department of
Public Instruction and Wisconsin Head Start, including Migrant and Sovereign Tribal
Nation Head Start programs.
