Fond du Lac County’s Collaborative Home Visitation Model

The mission of the “Safe and Stable Families” group in Fond du Lac County is to mobilize community resources to strengthen and support families. They obtained a state Prevention of Child Abuse and Neglect (POCAN) grant in 1998 that focuses on coordinating a continuum of home visiting services in Fond du Lac County. They changed their name to emphasize their positive approach to working with families. Their program is now called “Promoting Opportunities for Care and Nurturing” (POCAN).

My name is Diane Fett and I am the director of Fond du Lac's Birth-to-Three program. When asked whether there were counties in the state of Wisconsin that had models that others might learn from, I responded by saying that I thought we did. I think we are doing some good things in Fond du Lac, and wanted to share those with others.

Right now our Family Preservation and Support Group is called 'Safe and Stable Families'. Our mission is to mobilize community resources to strengthen and support families. The Pocan Initiative is a major part of that. We serve as an advisory board to their group, therefore, the focus has been on home visitation. We've expanded that to say 'Now how does that reflect a continuum of services, from very generalized services to very specific services for families?'. That's where Pocan comes in.

Home Visit Continuum

     
   
 

The POCAN grant, when we applied for it, really allowed us to think through home visitation services in our community, and to identify gaps that weren't being filled. We knew we had many strong partners in home visitation, and in our community, but each home visitation program was operating [independently], so the grant really enabled us to sit together at the table and create a standardized system of home visitation for all members in our community, and each family in Fond du Lac county receives home visitation through the most appropriate agency. So that's what really excited folks.
