Wisconsin Medical Home
- The Family Health Program in Wisconsin's Department of Health Services has the responsibility to develop capacity and infrastructure to assure that health care systems "work" for mothers, children and families statewide.
- Maternal and Child Health (MCH) Hotlines (includes Wisconsin First Step: information and referral hotline for children and adolescents age birth to age 21 with special needs and families and service providers).
- Wisconsin Regional Centers for Children and Youth with Special Health Care Needs can assist families is finding a medical home and assist health and service providers in strengthening medical home practices and policies.

- Wisconsin Medical Home Tool kit -This Toolkit includes interviews with parent and physician teams in the state who have worked to create strong Medical Homes. There are linkages to many resources that strengthen communication between families and providers so children receive quality, coordinated care.
- The Waisman Center in partnership with the Children and Youth with Special Health Care Needs Program, WI DHS has created a series of webcasts for parents, professionals and physicians related to a number of topics around child development, early identification of children with disabilities, and early intervention. Each webcast is about 20-30 minutes in length and presentation handouts are available. Viewers will be asked to take a quick survey prior to viewing the presentations.
