Wisconsin Medical Home

The Family Health Program in Wisconsin's Department of Health Services has the responsibility to develop capacity and infrastructure to assure that health care systems "work" for mothers, children and families statewide.
Maternal and Child Health (MCH) Hotlines (includes Wisconsin First Step: information and referral hotline for children and adolescents age birth to age 21 with special needs and families and service providers).
Wisconsin Regional Centers for Children and Youth with Special Health Care Needs can assist families is finding a medical home and assist health and service providers in strengthening medical home practices and policies.

Wisconsin Medical Home Tool kit -This Toolkit includes interviews with parent and physician teams in the state who have worked to create strong Medical Homes. There are linkages to many resources that strengthen communication between families and providers so children receive quality, coordinated care.
The Waisman Center in partnership with the Children and Youth with Special Health Care Needs Program, WI DHS has created a series of webcasts for parents, professionals and physicians related to a number of topics around child development, early identification of children with disabilities, and early intervention. Each webcast is about 20-30 minutes in length and presentation handouts are available. Viewers will be asked to take a quick survey prior to viewing the presentations.

National Medical Home

National Center for Medical Home Implementation (American Academy of Pediatrics 5-year cooperative agreement grant from the federal Maternal and Child Health Bureau).
Medical Home Implementation CME Series: Recordings Now Available! The AAP has been hosting a free CME series to provide child health professionals with practical strategies for implementing medical home in practice. Four events have been completed and with one more to go.

These informative calls/webinars were led by nationally recognized experts with the goal of educating participants about the value of the family-centered primary care medical home for all children and youth, the availability of practical tools and resources, and strategies for improving care and increasing patient/family satisfaction. The focus of Child Health Day 2009 was "Every Child Deserves a Medical Home." To recognize Child Health Day, the National Center for Medical Home Implementation produced a video compilation featuring children, families, and providers across the country who illustrate the value and importance of medical home for all children and youth. You can view the video and other resources from Child Health Day, including coloring sheets and flyers, on the National Center's Web site.
