  OSEP Child Outcomes - Requirements & Forms

A district is assigned a sample year beginning July 1 and ending June 30 of the following year. Districts assigned year are listed at the website:

Any children with an initial IEP between July 1 and June 30 of your district's assigned year is included in your "entry cohort" including "speech and language only" children. (Note: The initial placement signature date of the initial IEP must fall within this date range.)

 

A district develops a team process to: collect and organize information, use the Child Outcome Summary Form (COSF) within teams and make determinations about ratings. Input from parents must be included when gathering information on the child's level of functioning.

Using the COSF, the team determines the entry rating within 60 days of entry for each child.

Follow the entry cohort and complete the Child Outcomes Summary form for each child as the child exits services or is 6 by September 1st until all of the students in your entry cohort have been exited.
 

Maintain local records of the Child Outcomes Summary Form.  

The Child Outcomes Fidelity Self-Assessment is intended to be used by district and county personnel involved in the OSEP Child Outcomes Process (Indicators #3 Part C; #7 Part B). The items listed on the self-assessment are the activities carried out by districts and counties that ensure the work is being done with fidelity.
